CASE DESCRIPTION

Aurora Health Care, an integrated, not-for-profit, health care provider serving communities throughout eastern Wisconsin and northern Illinois, was looking for ways to engage expectant moms in the community who typically have low compliance to prenatal care.

Since Aurora was already using Babyscripts Schedule Optimization module, Aurora looked to us to develop and deploy the Care Navigator module for their socio-economic at-risk patient pregnant population in Milwaukee, WI.

THE OUTCOMES

Patients using Babyscripts Care Navigator attended 4 more visits across the pregnancy than the baseline average. Additionally, Babyscripts was able to identify and triage 2 risk factors, on average, per patient (e.g., domestic violence, nutrition and financial support, transportation, workplace harassment, etc.).
